Ishin University’s Sugai (Yasunori Danta) plans to make a comeback in the president of the Japan Medical Association race by succeeding in the operation of Chairman Uesugi (Masaaki Sakai) of Uesugi Motors, the top Japanese car manufacturer.

The key to the operation is the cutting-edge medical robot, Elcano Darwin, the next iteration of the Elcano. It was developed by Ishin University’s Dr. Reiko Hayakawa (Takiuchi Kumi) with the assistance of Tojo University’s Takashina (Koutaro Koizumi).

Sugai proposes to use “World’s Number One Doctor,” the Elcano Darwin, for Uesugi who requires a direct anastomosis. Uesugi demands for it to be controlled by the top doctor as well. In comes Amagi (Kazunari Ninomiya).

Sugai had requested Amagi to operate a clinical trial so that Elcano Darwin could learn how to do a direct anastomosis before Uesugi’s operation. Hearing Chairman Uesugi’s massive total assets, Amagi agrees.

Reiko, Sugai and the team arrives at Tojo Uni before the clinical trial where they meet Nekota (Shuri). Nekota used to be a nurse at Ishin Uni 8 years ago and had quit after causing an incident which Reiko brings up.

On the day of the clinical trial, Amagi suddenly announces to cancel the operation after seeing the state of the patient. On top of that, Reiko suddenly falls down clutching her chest…
